Registration
If you live within our practice area, you are eligible to register as a patient. You will need to complete some registration forms and will be required to make an appointment with the practice nurse for a simple health check. If you possess a NHS medical card please bring it along with you at registration.

Booking an appointment

From 1st July 2019 we will be operating a new appointment system. Under the guidance of the GP partners, reception staff have been trained to help navigate you to the most appropriate healthcare professional. You will be asked the nature of the problem and whatever you say will be treated in the strictest confidence. Routine appointments will be available one week in advance and urgent medical problems on the day will be triaged by the duty doctor. The doors will now be opened at 8.15am. The telephone lines are manned between 8am and 6.30pm Monday-Friday (with the maximum of 6 lines between 8am and 8.30am to cope with the demand). All routine appointments are for 10 minutes. Chronic Disease management and other more specialized procedures will require a double/triple appointment. Please let us know if you are unable to attend an appointment. Patients arriving late may be required to make an alternative appointment. If a chaperone is required please ask the receptionist at the time of booking the appointment.

Telephone Advice
Often problems can be dealt with by telephone. The receptionists may take your telephone number and ask an appropriately qualified person to ring you back. Please make sure that you leave a number that is accessible.

If you have an emergency that cannot wait until the surgery opens, please telephone the surgery on 20 461100.

Repeat Prescriptions
My Health On-line (MHOL) - if you are on regular repeat prescriptions and would like to request your prescription on-line please ask reception for further details.
Please note we do not accept repeat prescription requests over the telephone. You may obtain repeat prescriptions at reception giving 2 working days written notice. Alternatively, you may post your request enclosing a stamped addressed envelope or fax us on 20 451623.

Test Results
Test results will be available after 2pm. You do not need to speak to the doctor to find out the result of your test. The doctors will review the results and leave a message with the receptionists regarding appropriate action. You will be advised at the time of the test when the results will be available.

Registrar
The practice is a training practice. Qualified doctors who wish to specialise in general practice spend up to a year at a time, under supervision, at the practice.

Medical Research Data Collection
The staff at this practice record information about you and your health so that you can receive the right care and treatment. We need to record this information, together with the details of the care you receive, so that it is available each time we see you.
The information recorded about you may be used for reasons other than your personal care, for example, to help to protect the health of the general public, to plan for the future, to train staff and to carry out medical and other health research. We are involved in research studies which require access to anonymous information from patients’ notes. You cannot be identified from these notes as all personal details (name, address, post code, full date of birth) are removed. Individual patients’ records are added into a much larger anonymous database from many patients across the UK which is used by researchers outside this practice. This data may be anonymously linked to other data, such as hospital data.
If you would like to opt out of this data collection scheme, please let your doctor know and your records will not be collected for use in the anonymous research database. This will not affect your care in any way. If anything to do with the research would require that you provide additional information about yourself, you will be contacted by your GP to see if you are willing to take part; you will not be identified in any published results.
Some Research Studies use Docmail to send information to patients on behalf of the practice. Docmail comply with the Data Protection Act 1988 in particular with the obligations set out in the seventh principle of Schedule 1 Part 2 of the Data Protection Act 1998. Docmail will utilise recognised ‘best practice’ security features to protect your data and maintain ISMS27001:2013 data security registration to ensure that this continues to be the case. Docmail will only hold on to your data for as long as is necessary to complete your mailing. In any event all of the data will be deleted after 30 days.
You have a right of access to your health records. If at any time you would like to know more, or have any concerns about how we use your information, please ask reception for more details.
EVERYONE WORKING FOR THE NHS HAS A LEGAL DUTY TO KEEP INFORMATION ABOUT YOU CONFIDENTIAL.

Zero tolerance Policy
The practice operates a zero tolerance policy with regard to physical and verbal abusive behaviour towards any member of staff. The full policy is available on request.

Services Available

Well Baby Clinics
The health visitor holds a clinic every Tuesday between 2.00 and 4.00pm. All children of pre-school age are welcome to attend this clinic.

Child Health Surveillance
You will be notified when your baby is due for a routine health check. Please make an appointment for this via reception.

Family Planning
We offer a full range of contraceptive services during regular surgery hours. Follow-up appointments can be made with the practice nurse.

Cervical Cytology (Smear Test)
We strongly advise women, aged 25 and 64, to have this test every three years. A double appointment is required with the practice nurse or a female doctor.

Antenatal Clinic
The midwife holds an antenatal clinic on a Thursday afternoon from 1pm. The doctors at the practice can also provide antenatal care during normal surgery hours.

Minor Surgey
The surgery is well equipped for a range of minor surgical procedures. Minor surgery appointments can be booked in advance after consultation with a doctor.

Travel Vaccinations / Immunisations
The practice nurse is available for travel advice and immunisations. Please make an appointment to discuss your requirements well in advance of your intended date of travel.

Private Medical Examinations / Private Medical Claim Forms
Some services are not available on the NHS that occur a fee (link to pricelist). You will be required to pay in advance for completion of any private insurance claim forms.

What to do in time of Bereavement
If death occurs at home, telephone the doctor who will visit you to confirm death has taken place and will tell you what to do next.
